Community Stroke Service

The Tower Hamlets Community Stroke Team (THCST) provides multi-disciplinary assessment, rehabilitation and support for people with a diagnosis of a new stroke.

The team includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychology and Rehabilitation Support Workers. We also work closely alongside our Stroke Association Recovery Support Coordinator in Tower Hamlets and colleagues within Tower Hamlets social services.

Team members work collaboratively with service users and their family members, friends and carers to provide individualised goal-based rehabilitation and support. We see people in their own home, in clinic at the Royal London Hospital, or in other community settings, as appropriate and depending on rehabilitation needs and service user preference.

We aim to be flexible and responsive as a service, while also encouraging self-management principles for our service users. Goals are reviewed regularly with service users and their family/carers, to support engagement in rehabilitation. Our team can also provide tailored information provision and advice to service users and their family, friends and carers. This may also include signposting and making onward referrals to other relevant teams, services and organisations who provide support after stroke.

Service and eligibility criteria

Referrals are accepted for people who have

  • Received a diagnosis of a new stroke, within the last 12 months
  • Are aged 18 or over
  • Are currently living in Tower Hamlets 

It is preferable, but not essential to have a GP in Tower Hamlets

Referral form information should include appropriate therapy goals, where possible.

Please note: If an individual’s stroke diagnosis is over 12 months, please refer to the Tower Hamlets Community Neuro Team (THCNT) via the SPA form

How to refer

Service-users should be referred by the  TH CST Referral Form  Referral forms should be sent via email to: bartshealth.THCST@nhs.net

Our service includes the following referral pathways: 

  • ESD (Early Supported Discharge)
  •   CSR (Community Stroke Rehabilitation)

If you have previously accessed our service or recently moved into Tower Hamlets and feel you require further support, advice, information and input from our team, (and are within 12 months of your stroke), please contact the team to discuss making a self-referral on 020 3594 1156.
