New trial targets high blood pressure

At St Bartholomew's Hospital we're trialling a new device that reshapes the carotid sinus to reduce blood pressure while maintaining normal blood flow.

A new clinical trial, called CALM-2 (Controlling And Lowering blood pressure with MobiusHDÒ), sponsored by California-based Vascular Dynamics, Inc. (VDI), is a novel endovascular approach to treat patients with drug-resistant high blood pressure.

High blood pressure, or hypertension, is recognized as the most important single risk factor for cardiovascular morbidity and death. Despite advances in the effectiveness of medications, many people struggle to control their blood pressure to targeted levels, despite antihypertensive medications. For these people and their loved ones, the need for a non-pharmaceutical treatment has become of critical importance. 

Specialized stretch-sensitive nerves, called baroreceptors, are located in the walls of the carotid arteries and play an essential role in the body’s natural blood pressure regulation. Vascular Dynamics has developed the first catheter-based, non-surgical approach that utilizes the baroreceptor mechanism to address uncontrolled hypertension. This approach includes placement of the MobiusHD implant, a thin rectangular-shaped device designed to reshape the carotid sinus to increase baroreceptor stretch and amplify nerve signaling to the brain. This serves to reduce blood pressure while maintaining normal blood flow and beat-to-beat fluctuations.

“The consequences of high blood pressure represent a tremendous unmet clinical need, and a non-pharmaceutical solution is needed,” said Principal Investigator Professor Melvin Lobo from Queen Mary University of London and Barts Health NHS Trust.

“The initial results using the MobiusHD implant to reshape the carotid sinus and stretch baroreceptors to trigger the body’s natural blood pressure regulation is impressive and of great interest to us. The safety and efficacy for this approach must be confirmed in a larger clinical trial, and we look forward to being an integral part of this important study.”

Enrollment for the CALM-2 study has begun at select medical centers around the UK, Europe and the U.S. with Professor Lobo leading these efforts at St Bartholomew’s Hospital.

The MobiusHD device was studied in an earlier proof-of-concept study, and results were published in 2017, showing significant reductions in blood pressure through six months after implantation.  These encouraging results led to the design of the larger multi-center clinical trial, CALM-2, intended to generate definitive evidence of safety and effectiveness of this approach.

    Hello!     I am a 69yo female, I have had high blood pressure for a number of years,about 16years ago I had a heart attack at the time I had insurmountable stress that contributed to my heart attack test proved I had no blockages so was put on medication to reduce my blood pressure. My blood pressure has never been controlled.i am about to add a third drug for high blood pressure that I am reluctant to do. I have family history of high blood pressure, with deverstating consequences. Would like to find an alternative to drugs hence why I am in touch.

    Like many of the other posts, I heard your advert on LBC yesterday and would be interested in getting involved in the trial. I am 68 years old, cycle about 120 miles a week, and have had high blood pressure since 1996 at least. It's on the borders of being controlled at around 140/80, but I've tried many combinations of drugs - most of which have had unacceptable side effects.

    I am 67 year old Male. I have been on 20mg Lecandipine for 2 years after a sudden spike in BP which lasted one day. Atrial Flutter occurred three months later. I have a slow pulse rate so a pacemaker was fitted a year ago to act as a barrier should I need a beta blocker. Flutter is very infrequent. Recently for some reason I had another spike in BP. This has resulted in 2.5 mg of Ramipril added to the “mix”
    I would be very pleased to be part of this trial. I live in South Wales and will cooperate fully.
    I believe my enthusiasm and willingness would be an asset.

    Despite taking six medications for hypertension I still regularly have bp readings of over 200. Ironically, on waking the readings can start at 187 which, within 1 1/2 hours will drop to around 96. I would very much welcome further tests to ascertain the cause of this problem.
