ARCaRe

Welcome to the Barts Health Respiratory Care and Rehabilitation Service (ARCaRe). We provide care, support and rehabilitation exercise and education for patients with respiratory problems livening in Tower Hamlets.

We work closely with GPs and our hospital staff to provide the care and treatment that is best for patients’ needs.

About this service

Patients are cared for at home, in a number of community halls around Tower Hamlets and in hospital. 

Enhanced care

We provide enhanced care to patients through individual case management by respiratory specialist staff for patients with two or more admissions to hospital because of an exacerbation of respiratory disease in the previous 12 months. We aim to empower patients to feel more confident to self-manage their condition, as well as looking at lifestyle and support changes that may improve quality of life.

Inpatient / supported discharge

We carry out specialist respiratory reviews for patients in hospital and identifying if they can be discharged earlier and given extra specialist support in the community. We can support patients at home for up to two weeks after discharge and hopefully prevent avoidable readmission to hospital.

Admission avoidance

Short term enhanced support is given to patients experiencing an exacerbation of their Chronic obstructive pulmonary disease (COPD). In some cases, giving this support can avoid the patient being readmitted to hospital. We aim to provide the right specialist care in the most appropriate setting.

Pulmonary and heart failure rehabilitation

We run an eight week course of exercise and education for patients with confirmed respiratory or heart failure diagnoses who are limited by breathlessness. Research shows that pulmonary rehabilitation, alongside stopping smoking, are two of the most effective treatments that are available for patients with COPD. For optimum benefit, patients should attend twice a week. Our venues for classes are across Tower Hamlets in a mixture of community and hospital settings, so there is plenty of choice for patients.

When we receive a referral, we will make an appointment to assess the patient and make time to discuss the options then. We aim to make the classes both enjoyable and informative. Aims to reduce breathlessness and support positive lifestyle changes.

Accessing this service

Our community services are available to patients with diagnosed respiratory conditions (and heart failure for rehabilitation) who are currently living in Tower Hamlets and registered with a Tower Hamlets GP.

Enhanced care

Enhanced care referrals can be made by a health professional using the generic referral form to the Single Point of Access. We aim to assess patients within four weeks of referral.

Inpatient / supported discharge

Patients are automatically identified daily from Emergency Department admission lists.

Admission avoidance

Referrals to admission avoidance can be made by GPs, practice nurses and outpatient clinics. We aim to make contact with the patients within 24 hours of receiving the referral.

Pulmonary rehabilitation

Referrals can be made by any health professional directly to the ARCaRe office either by fax or email. Patients are encouraged to discuss with their GP’s / practice nurses if they feel they meet the criteria for referral

Who will I see?

Our team is formed of:

  • Physiotherapists
  • Nurses
  • rehab support workers
  • occupational therapists
  • psychologists
  • dieticians

We work together to provide advice and clinical expertise for patients with respiratory conditions.

Pulmonary rehabilitation sessions are supervised by a qualified physiotherapist and an assistant. We aim to have a Bengali speaking member of staff available to help support Bengali patients who do not speak English with the exercise and education.

Location of the Respiratory Care and Rehabilitation Services

We have offices at Mile End and The Royal London hospitals, but our clinical work takes place in patients’ homes, hospitals and various community halls around Tower Hamlets.

Enhanced care referrals

Patient should be referred using the single point of access form and emailed to btl-tr.chsreferrals@nhs.net or chs_ooh@bartshealth.nhs.uk Alternatively call 020 3594 0942 to speak to a member of the team.          

Admission avoidance

Patients should be referred to the service using the CHS referral form, but to expedite a review we request that the referrer telephones 079 8317 7719. Our Team aims to make contact with the patient within 24 hours of receiving the referral.

Pulmonary rehabilitation

Patients referred for pulmonary rehabilitation should be referred by email to ARCaReCHS@bartshealth.nhs.uk or faxed to 020 8223 8509.
